General Privacy Policy
This practice
only collects information that is necessary for the provision of good
physiotherapy care.
The information
we gather is to assist us in providing “best practice” physiotherapy and only
the relevant parts of this information are discussed with or disclosed to your
general practitioner, specialist or case manager. Occasionally, information
about patient consultation is provided to our insurance underwriters, in
relation to current or potential insurance claims. If there is a need to
disclose information about your case with people other than those directly
associated with your treatment we seek your consent first.
We are mindful of
the need to ensure that the information we keep on record is accurate and
complete to the date of the last contact with the practice. All material is
treated with full regard to the privacy of our patients.
Occasionally this
practice may be involved in research into health issues. If any data from this
practice is used in this manner it will not include information which identifies
from whence it came.
Clinical records
at this practice are sometimes used for quality assurance or clinical audit
activities which are undertaken to help improve the treatment and service we
provide. Data used in these projects has no identity.
Reasonable steps
are taken to protect the records we hold from misuse, loss, unauthorised access,
modification or disclosure. Nobody other than our staff has access to this
information. As space dictates older records are archived in a secure
location. Record destruction is performed with Privacy Act compliance.
Intouch Physio is
bound by the Privacy Amendment (Private Sector) Act 2001, and operates in
accordance with the Code of Conduct of the Australian Physiotherapy Association.
If you would like
to discuss any aspects of our privacy policy, or review your health records,
please advise your treating physiotherapist. We are pleased to discuss our
procedures with you and to amend any inaccuracies in your records.
